ABSTRACT

Purpose To explore the expression of CBX8 in gastric adenocarcinoma tissue and its clinical significance.Methods 119 cases of primary gastric adenocarcinoma treated by surgery and corresponding adjacent normal tissues were collected. The protein expression levels of CBX8 in gastric adenocarcinoma and their corresponding adjacent normal tissue was determined using tissue microarray and immunohistochemistry PV 6000 method staining. The relationship between CBX8 expression and clinicopathologic characters and survival prognosis was analyzed. Results The positive expression rate of CBX8 in gastric cancer and their corresponding adjacent normal tissue was 46.2% (55/119) and 13.4% (16/119) respectively. There was statistically significant difference between two groups (χ2=30.53, P < 0.01). The expression of CBX8 in gastric adenocarcinoma tissue was obviously correlated with the differentiation, clinical staging, and lymph node metastasis (P < 0.05). The high expression rate of CBX8 in the poorly differentiated gastric adenocarcinoma was lower than moderately differentiated and well differentiated group. The high expression rate of CBX8 in stage Ⅰ + Ⅱ gastric adenocarcinoma was higher than that in stage Ⅲ + Ⅳ. The gastric adenocarcinoma patients with high expression of CBX8 had low metastasis rate. Cox multivariate analysis showed CBX8, clinical staging, and lymphatic metastasis were independent predictors of overall survival and disease-free survival (P < 0.05). Kaplan-Meier analysis showed that the patients with higher expression of CBX8 had both longer overall survival time (P = 0.004) and disease-free survival time (P =0.004). Conclusion The expression of CBX8 may be involved in the tumorigenesis and progression of gastric adenocarcinoma. CBX8 is one of the independent predictor of prognosis, and the detecting of CBX8 expression may have important clinical value for the evaluation of gastric adenocarcinoma. CBX8 may became a new target for target therapy of gastric adenocarcinoma.

ABSTRACT

Purpose To investigate the value of Cyclin D1 and claudin 7 expression in the differential diagnosis of renal epithelial tumors.Methods The expression of Cyclin D1 and claudin7 was detected by immunohistochemical staining with tissue microarray.Totally 309 cases of renal epithelial tumors and 20 cases of normal renal tissues were collected for immunohistochemical staining.Comparative analysis for the values of Cyclin D1 and claudin 7 was performed on the diagnosis and differential diagnosis of renal tumors.Results The positive rates of Cyclin D1 in renal oncocytoma (RO),chromophobe renal cell carcinoma (ChRCC),clear cell renal cell carcinoma (CCRCC),papillary renal cell carcinoma (PRCC),Xp11.2 translocation/TFE3 gene fusion associated renal cell carcinoma (Xp11.2/TFE3) and clear cell tubulopapillary renal cell carcinoma (CCTPRCC) were 86.2% (50/58),8.2% (4/49),70.0% (98/140),8.8% (3/34),42.9% (6/14),and 71.4% (10/14),respectively.The positive rates of RO were significantly different from those of ChRCC and PRCC (x2 =64.72,52.56,P <0.000 1).Significant differences in positive rates of CCRCC and ChRCC (x2 =55.87,P < 0.000 1),PRCC and Xp11.2/TFE3 (X2 =4.28,P=0.039),CCTPRCC and ChRCC (x2 =21.69,P <0.000 1)were also observed.The positive rates of claudin 7 in RO,ChRCC,CCRCC,PRCC,Xp11.2/TFE3 and CCTPRCC were 20.7% (12/58),87.8% (43/49),8.6% (12/140),50%(17/34),14.3% (2/14),and 57.1% (8/14),respectively.There were significant differences in positive ratios of RO and ChRCC (x2 =47.82,P < 0.000 1),CCRCC and CCTPRCC(x2 =26.57,P <0.000 1),PRCC and Xp11.2/TFE3 (x2 =5.29,P < 0.05).The sensitivity and specificity of Cyclin D1 +/claudin 7-immunophenotype for RO were 69% and 95.9% respectively.The diagnostic sensitivity and specificity of Cyclin D1-/claudin 7 + for ChRCC were 83.7% and 96.6%.In the identification of CCRCC and CCTPRCC,the sensitivity and specificity of Cyclin D1 +/claudin 7-for CCRCC were 65.7% and 71.4%,and the diagnostic sensitivity and specificity of Cyelin D1 +/claudin7 + for CCTPRCC were 42.9% and 95%.Conclusion The differential expression of Cyclin D1 and claudin7 in RO and ChRCC,CCRCC and ChRCC,PRCC and Xp11.2/TFE3 suggests that combined detection of two proteins provides an important assistance for the identification of these renal epithelial tumors.

ABSTRACT

<p><b>OBJECTIVE</b>To compare the prognostic value of clinical risk score and thrombolysis in myocardial infarction (TIMI) flow grade alone or combined on outcome of acute coronary syndrome (ACS).</p><p><b>METHODS</b>A total of 206 eligible patients [135 males, mean age (67.57 +/- 9.88) years] were enrolled. The primary endpoints included cardiac death and non-cardiac death. The secondary endpoints included non-fatal stroke, reinfarction, heart failure and recurrent angina. Receiver operating characteristic curve (ROC) established by using different endpoints and clinical risk score, TIMI flow grade or combined risk scores. The prognostic value for different endpoint expressed as the area under the curve (AUC).</p><p><b>RESULTS</b>Eleven patients lost during the (11.41 +/- 5.33) months follow up and data were available for 195 patients, 8 patients reached the primary endpoints, and 17 patients reached the secondary end-points at the end of follow up. The AUC was 0.67 (95% CI = 0.557 approximately 0.786), P = 0.006; 0.68 (95% CI = 0.557 approximately 0.786), P = 0.004 and 0.730 (95% CI = 0.691 approximately 0.815), P < 0.001, respectively for clinical risk score, TIMI flow grade and the combined risk score respectively. There were no significant differences among clinical risk score, TIMI flow grade and combined risk score (all P > 0.05) for AUC and for primary end point and the secondary end point.</p><p><b>CONCLUSION</b>The result from this study suggests that the efficacy of predicting the total events based on clinical risk score, TIMI flow grade and combined risk score was similar.</p>

ABSTRACT

<p><b>OBJECTIVE</b>This study was designed to compare clinical efficacy of segmental pulmonary vein ablation (SPVI), amiodarone or amiodarone plus losartan on sinus rhythm maintenance in patients with lone paroxysmal atrial fibrillation (PAF).</p><p><b>METHODS</b>Patients with lone PAF were treated with amiodarone alone (A, n = 52), segmental pulmonary vein isolation (SPVI, n = 51), or amiodarone plus losartan (AL, n = 51). The primary endpoint of this study was the incidence of symptomatic atrial tachyarrhythmia (> 30 s) documented by 12 lead ECG or Holter during 12 months follow-up period.</p><p><b>RESULTS</b>During follow-up, AF was documented in 24 patients (46.2%) in A group, 11 patients (21.6%) in SPVI group and 12 (23.5%) in AL group (P < 0.05 vs. A group). The Kaplan-Meier survival analysis demonstrated a significant equally reduction in AF recurrence in SPVI and AL groups (P = 0.009, log-rank test and P = 0.018, log-rank test, respectively) compared with A group. The hazard ratio for AF recurrence in patients treated with SPVI and amiodarone plus losartan was 0.41 (95% CI 0.200 to 0.848, P = 0.016) and 0.46 (95% CI 0.225 to 0.953, P = 0.036), respectively. Incidences of major adverse cardiac events were similar among the groups (9.6% in A, 3.9% in SPVI and 7.8% in AL group, P > 0.05).</p><p><b>CONCLUSION</b>The results of this study suggest that the segmental pulmonary vein isolation and amiodarone plus losartan are superior to amiodarone alone for preventing AF recurrence in patients with lone PAF.</p>

ABSTRACT

<p><b>OBJECTIVE</b>The aim of the present study was to detect the expression of calpain-I, calpastatin, caspase-3 and apoptosis in the left atria of patients with rheumatic heart disease (RHD), and to find the association of these factors. Also, it was intended to investigate the effect of the above factors on the mechanism of atrial fibrillation (AF).</p><p><b>METHODS</b>43 patients with RHD undergoing valve-replacement were included, 15 patients with regular sinus rhythm (Group RSR), 8 patients with paroxysmal AF (Group AF1) and 20 patients with permanent AF (Group AF2). Western blot was used to examine the content of calpain-I, caspase-3 and calpastatin. The apoptosis index (AI) was measured by TUNEL.</p><p><b>RESULTS</b>(1) Expression of calpain-I in group AF2 was increased to (344.0 +/- 101.9)%, and caspase-3 was increased to (394.0 +/- 99.4)% compared to group RSR (P < 0.01, respectively). Amount of calpastatin was reduced to (27.0 +/- 12.8)% (P < 0.01). The expressions of these proteins were unchanged in group AF1. (2) AI in group AF2 was higher than that in groups RSR and AF1 (P < 0.01). (3) In group AF2, the levels of calpain-I, caspase-3 and AI were positively relative to left atrial dimension and AF duration, P < 0.05 - 0.01, respectively, whereas calpastatin was negatively correlated with left atrial dimension and AF duration (P = 0.007 and P = 0.001, respectively). (4) The protein content of calpain-I was positively related with that of caspase-3 and AI (P < 0.01, respectively), and the content of calpastatin was negatively related with that of calpain-I and caspase-3 (P < 0.01, respectively).</p><p><b>CONCLUSIONS</b>Apoptosis of atrial cell increased in left atria and the protein contents of calpain-I, caspase-3 and calpastatin significantly altered during AF in humans with RHD. The observed interactions suggest that these factors compose a system to cause the structural remodeling and dysfunction of atria. The course may play a key role in promoting the onset and maintenance of AF.</p>
